Qualifications

• Graduate of an accredited nursing program.
• Current RN or LPN license in state of practice.
• 1–2 years of clinical nursing experience preferred.

Responsibilities

• Provide clinical assessment and triage for patients during walk-in visits and appointments.
• Utilize guidelines and protocols to determine acuity and recommended care (clinic visit, urgent care, ER referral, or home management).
• Document triage assessments, advice, and follow-up plans in the EHR.
• Collaborate with providers and clinic staff to ensure timely and appropriate care.
• Provide patient education regarding symptoms, medications, follow-up care, and self-care strategies.
• Identify and elevate urgent or emergent situations appropriately.
• Assist in care coordination by scheduling appointments, referrals, and transportation.
• Maintain confidentiality and HIPAA compliance.
• Participate in quality improvement initiatives and staff training.
• Support clinic flow and provide direct patient care as needed.
