Job summary

Heartland Family Service in Papillion, NE is seeking a Triage Navigator to be the first contact for consumers. The role involves screening referrals, orienting clients to the CSRC program, assessing immediate crisis stabilization needs, and guiding individuals to appropriate supports.

The position supports trauma-informed, culturally competent care, works 40 hours per week overnight from 10 p.m. to 8 a.m., and emphasizes teamwork and client-centered services.

Qualifications

  • Two years direct care experience in human service field; two years of training in human service field or a bachelor’s degree or higher in psychology, sociology or related human service field which is preferred.

Responsibilities

  • Complete enrollment/intake on eligible clients
  • Develop a supportive relationship with clients during scheduled shifts
  • Orient clients to CSRC program expectations and address non-compliance to ensure a safe environment
  • Psycho-educate clients on life, social and coping skills
  • Create, maintain and facilitate in-house client life- and social-skill activities and support groups
  • Provide problem solving and crisis intervention services to empower clients to develop self-sufficiency
  • Monitor client’s self-administration of medication and follow procedures for recording
  • Provide reports to CSRC staff regarding client activities and needs at shift change
  • Demonstrate knowledge of current community and HFS programs and resources
  • Utilize trauma-informed care, person-centered and culturally competent principles in providing services to clients
  • Complete client room safety checks and searches
  • Assist with donation disbursement for discharging clients
  • Responsible for cleanliness and upkeep of facility including staff offices and client areas
  • Place maintenance requests for facility repairs
  • Respond to emails, phone calls, and faxes professionally
  • Work independently or as part of a team
  • Maintain courteous and caring attitude toward clientele, volunteers, and visitors

SUMMARY DESCRIPTION OF WORK

Triage Navigators will be the first point of contact for consumers. When a referral is received into the facility, a triage navigator will complete a screening form which is utilized to determine how well the individual’s needs and requests match Crisis Stabilization and Resource Center’s services and levels of care.

The Triage Navigator will introduce and orient the consumer to the program, ensure the consumer understands their rights and responsibilities, and begin to assess the consumer’s immediate crisis stabilization needs. Triage navigation staff will supervise and be available for all mental health needs of the individual as they remain in triage.

Navigators will assist the consumer in identifying triggers and learn coping skills to utilize in times of stress and crisis. Triage Navigators will assist consumers in creating their initial stabilization plan to include connections to their natural supports and resources to help them resolve their crisis in the least restrictive environment.

Compensation:between $20.42 and $24.70 per hour (wage is based on total years of relevant experience)
Work Schedule:40 hours per week (overnights, 10:00 p.m. to 8:00 a.m.), Sunday Monday, Tuesday, and Wednesday

MINIMUM QUALIFICATIONS

  • Able to work both independently, with little supervision, and as a part of a team
  • Always display a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency
  • Abide by all specific program and Agency procedures, policies, and requirements
  • *Is dependable and punctual regarding scheduling and attendance
  • Cooperate and collaborate with program area staff, volunteers, and other Agency staff
  • Develop personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media
  • *Create, maintain, and share as appropriate a dynamic self-care plan
  • *Strive to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • *Essential functions of this job are to be performed on company physical work site.
