Job summary

Compass Connections is seeking an experienced registered nurse to provide telehealth triage and in-person assessments in Texas. You must hold a BSN and a Texas RN license with at least 5 years of RN experience; pediatric nursing is preferred.

Fluency in English and current BLS, CPR, and First Aid certifications are required. Responsibilities include accurate documentation of medical findings, communication with the Medical Director and RN Managers, and strict adherence to HIPAA/PII protocols and

Qualifications

• RN license in Texas with good standing.
• BSN degree required.
• Minimum 5 years RN experience; pediatric experience preferred.
• Fluent in English; strong written and verbal communication.
• Current BLS/CPR/First Aid certifications.
• Ability to perform telehealth and in-person assessments per protocol.
• Accurate medical documentation and HIPAA compliance.
• Successful background checks and immunization proof.

Responsibilities

• Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
• Provide telehealth triage services to assess patient needs and evaluate symptoms; instruct and educate patients.
• Conduct in-person and virtual assessments per protocols and document medical findings accurately.
• Gather, collect, and clarify patients' current condition and past medical history.
• Communicate findings with Medical Director as needed for patient health concerns.
• Coordinate with RN Managers and other staff to develop care plans and provide nurse support.
• Implement Compass Connections safety protocols including emergency evacuations with children and staff.
• Maintain confidentiality of health care records; HIPAA and PII compliance.
• Address other duties as assigned to maintain a fully functional program.

Work experience

Critical Action Items & Measurable Deliverables:

1.Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
2. Provide telehealth triage services to assess patient needs and evaluate symptoms. This includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
3. Conduct in-person and virtual assessments based on established protocols and accurately document medical findings.
4. Gather, collect, and clarify patients’ current condition and past medical history.
5. Effectively communicate with Medical Director as needed for reporting findings which are concerning for patient’s health and well-being.
6. Communicate with RN Managers and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.
7. Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
8. Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
9. Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.

Other Responsibilities:

1. Plan, organize and, prioritize daily work using sound judgement.
2. Monitor established health care plans for service population at the sites within the timeframe established by our State and Federal partners.
3. Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and standards, and program timeline requirements.
4. Meet all program and federal partners’ documentation and processes requirements.
5. Other duties as assigned.

Requirements:

1. Pass a pre-employment drug screen and random drug screens throughout employment.
2. Provide proof of work eligibility status upon request.
3. Pass a pre-employment and biennial criminal background checks.
4. Demonstrate the ability to:
   a. Work collaboratively with other staff members, service providers and professionals.
   b. Communicate effectively in writing and verbally in English.
   c. Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times.
   d. Maintain computer literacy required to meet the responsibilities of the position.
   e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
5. Demonstrate a working knowledge of all Compass Connections policies and procedures.
6. Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for the following:
   a. MMR
   b. Varicella
   c. Hep B
   d. Hep A
   e. Tetanus
   f. PPD
   g. Meningococcal (preferred)
7. Must be able to perform physical activity such as extensive walking, stretching, and bending.
8. Must be able to work in a team environment.
9. Must be able to maintain professional people skills, including oral, written, listening, and non-verbal communication skills.
