Position Overview

Certifications: Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention

Work Experience: Required – At least one year of experience as a RN; Preferred – pediatric nursing experience

Key Responsibilities
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Provide triage services to assess patient needs, evaluate symptoms, answer patient inquiries, provide patient teaching, and determine the urgency and appropriate course of action.
  • Have extensive knowledge of medical documentation to generate, decipher, and correct documentation methods in accordance with state and federal guidelines.
  • Perform nursing duties in compliance with all regulatory authorities and program protocols: active and ongoing nursing assessments, charting, scheduling appointments, routine observations, isolation observations, wellness checks, immunizations, medication administration, and time internal reports, when needed.
  • Ensure patient safety by assessing and monitoring health conditions, assist with procedures, operate medical equipment, administer vaccines, medications, and treatments ordered by healthcare provider, monitor for side effects and reactions, and provide teaching to patients and staff.
  • Arrange and coordinate appropriate health services, including urgent, emergent, and preventative health services for children in care.
  • Conduct health assessments based on established protocols and document findings accurately, including validating systems of record for required facility documentation and reporting to meet agency, federal, and state guidelines within required timeframes.
  • Supervise, direct, and delegate when appropriate to execute medical services reflected in healthcare plans and treatment orders, ensuring all orders directed by the prescribing healthcare professional are carried out as ordered and monitoring patient improvement.
  • Gather, collect, and clarify patients’ current condition and past medical history.
  • Communicate with pharmacies on ordering prescription medications, check medication deliveries for accuracy and compliance, and remediate discrepancies.
  • Maintain adequate over‑the‑counter medication supplies for each service population facility and arrange proper disposal of unused and expired medications.
  • Maintain documentation of controlled drugs, including reconciliation of all scheduled drugs used in the facility through the facility’s protocols and procedures.
  • Administer medication as ordered by the prescribing healthcare provider and complete accurate and timely documentation in electronic medical records and the Medication Administration Record.
  • Effectively communicate with Certified Nurse Practitioner and Lead Medical Coordinator as needed for reporting findings concerning patient health and well‑being.
  • Communicate with Lead Medical Coordinator and other medical staff to devise individual care plans and act as a nurse support to other medical staff.
  • Implement Compass Connections safety protocols, including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
  • Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.

  • Monitor established health care plans for the service population at the sites within the timeframe established by our State and Federal partners.
  • Know and recognize the signs of abuse, neglect, exploitation and sexually provocative behavior and implement professional response, reporting and documentation procedures.
  • Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and program timeline requirements.
  • Meet all program and federal partners’ documentation and process requirements.
Additional Responsibilities

Requirements
  • Pass a pre‑employment drug screen and random drug screens throughout employment.
  • Provide proof of work eligibility status upon request.
  • Pass a pre‑employment and biennial criminal background checks.
  • Demonstrate the ability to:
    • Work collaboratively with other staff members, service providers and professionals.
    • Communicate effectively in writing and verbally in English.
    • Work in a fast‑paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times.
    • Maintain computer literacy required to meet the responsibilities of the position.
    • Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
  • Demonstrate a working knowledge of all Compass Connections policies and procedures.
  • Provide current physical‑medical release and proof of immunizations, titers, or declinations for:
    • MMR
    • Varicella
    • Hep B
    • Hep A
    • Tetanus
    • PPD
    • Meningococcal (preferred)
  • Be able to perform physical activity such as extensive walking, stretching, and bending.
  • Work in a team environment.
  • Maintain professional people skills, including oral, written, listening, and non‑verbal communication skills.
