Job summary

Wabasso Rehabilitation & Wellness Center is seeking a Treatment Nurse focused on wound care and skin integrity to support residents’ healing and well-being.

This role is ideal for an LPN or RN who values teamwork and resident-centered care. You will perform wound and skin treatments, maintain accurate documentation, and support the nursing team in delivering high-quality care.

Qualifications

• Current Minnesota LPN or RN license in good standing.
• Wound care or treatment experience preferred (training available).
• Strong assessment, documentation, and communication skills.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Complete all wound and skin treatments per physician orders and facility protocols.
• Monitor and document wound progress, including staging, measurements, and interventions.
• Communicate changes in condition to the charge nurse, DON, and provider as appropriate.
• Maintain wound logs and ensure timely, accurate documentation in the medical record.
• Participate in weekly skin rounds and collaborate with the wound physician and interdisciplinary team.
• Assist with other nursing duties as assigned to support overall resident care.
