Job summary

Must hold TX LVN/RN license, BLS, 1 year in skilled nursing, and prefer wound care or IV therapy certs; strong communication and regulatory knowledge; on-site role with opportunities for growth.

Qualifications

• Valid LVN or RN license in TX is required.
• Current BLS certification is required.
• Minimum of one year of nursing experience in a skilled nursing or long-term care setting.
• Strong knowledge of nursing procedures, treatments, and medication administration.
• Excellent communication with patients, families, and health-care team members.

Responsibilities

• Administer prescribed treatments and medications according to protocols and orders.
• Monitor and assess patients’ health status, reporting changes promptly.
• Maintain detailed patient records, treatment logs, and progress notes.
• Collaborate to develop, implement, and update individualized care plans.
• Educate patients and families on treatment procedures and medication management.
• Ensure compliance with regulatory requirements and facility policies.
• Assist with admission, transfer, and discharge processes.
• Participate in quality improvement initiatives and staff training.
