Job summary

Brownsville Nursing and Rehabiliation Center is seeking a Treatment Nurse to deliver care focused on skin integrity for residents, based on assessments and individualized care plans. You will perform timely wound dressing changes, monitor healing progress, and document findings for QA and care planning meetings.

The ideal candidate holds a Texas RN/LVN license, excels in communication and documentation, and is willing to participate in skin care training and on-call rotations as required.

Qualifications

• Must have attended a nursing program at an accredited college, university, or vocational school.
• Must have an active Texas RN/LVN license.
• Wound Care Certification is preferred.

Responsibilities

• Provide quality of care to prevent and promote healing of skin integrity alterations.
• Communicate with DON/designee about skin integrity issues.
• Dress wounds and ensure timely dressing changes as prescribed.
• Monitor wound healing and suggest changes as needed.
• Perform and record weekly pressure ulcer assessments and measurements.
• Document weekly Skin Assessment results and share with QA and care plan meetings.
• May assist with other nursing functions as needed.
• Participate in on-call rotating schedule.

Primary ResponsibilitiesThe Treatment Nurse will provide quality of care to prevent and promote healing of alterations in skin integrity of each resident as determined by resident assessments and individual plans of care.Essential FunctionsCommunicate with DON/designee any alterations in skin integrity of each residentParticipate in training and in-servicing related to skin careDresses wounds and ensures that dressing changes are done timely as prescribedMonitors the progress of wound healing and makes recommendations for changes if neededPerforms and records weekly pressure ulcer assessment and measurement documentationCommunicates with responsible party and attending physician on resident care issues related to skin integrityDocument on Weekly Focused Skin Assessment the results of each resident’s audit and provide copies to appropriate facility personnel during QA and care plan meetingsMay be asked at times by DON or Administrator to help with other nursing functions Participate in facilities rotating nursing on-call scheduleEducational/Training RequirementsMust have attended a nursing program at an accredited college, university, or vocational schoolLicensing RequirementsMust have an active RN/LVN license in the state of TexasPreferred to have Wound Care CertificationExperience RequirementsPrevious experience in skilled nursing is preferredPhysical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
