Job summary

Northvine Post Acute is seeking a compassionate Licensed Vocational Nurse (LVN) to join our wound care team as a Treatment Nurse. You will perform advanced wound care, assessments, and interventions to promote healing for residents and patients.

In this role, you will collaborate with physicians, nurses, and the care team to deliver high-quality care, document evidence-based care plans, and educate families on wound management. CA LVN license and wound care certification are preferred.

Qualifications

• Current, unrestricted LVN license in CA.
• Wound care certification highly desirable.
• Strong clinical assessment and critical thinking.
• Proficiency in wound care techniques and wound vac therapy.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Conduct wound assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Perform advanced wound care treatments, including wound debridement, dressing changes, and wound vac therapy, according to physician orders and established protocols.
• Monitor wound healing progress and adjust treatment plans as necessary.
• Educate patients, families, and caregivers on wound care techniques, prevention strategies, and self-care practices.
• Document all assessments, interventions, and outcomes accurately and timely in the electronic medical record.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care coordination and continuity of care.
