Job summary

Northvine Post Acute is seeking a skilled Treatment Nurse (LVN) specializing in wound care. You will provide advanced wound care, assessments, and interventions to promote healing for residents and patients.

Responsibilities include assessing wounds, creating care plans with the care team, performing debridement and VAC therapy, and educating families on wound care. Strong communication and documentation are essential.

Qualifications

• Licensed LVN in CA with current, unrestricted license.
• Wound care certification preferred.
• Strong clinical assessment and critical thinking required.
• Excellent communication and interpersonal skills essential.

Responsibilities

• Conduct thorough wound assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Perform advanced wound care treatments including debridement, dressing changes, and wound vac therapy per physician orders.
• Monitor wound healing progress and adjust treatment plans as needed.
• Educate patients, families, and caregivers on wound care techniques, prevention strategies, and self-care practices.
• Document all assessments, interventions, and outcomes accurately and timely in the EMR.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care coordination.
