Job summary

OPCO Skilled Management is seeking a registered nurse leader to coordinate the center's wound care program in Albuquerque. The role oversees the wound team, care planning, and documentation while driving prevention and quality improvement.

Qualifications

• Current in-state RN/LVN/LPN license is required.
• One year of nursing management experience is preferred.

Responsibilities

• Coordinate and lead wound team rounds and processes; oversee care plans and documentation.
• Identify and monitor pressure ulcer and skin condition information for the center.
• Collect, analyze, and share pressure ulcer data with staff and clinicians.
• Investigate center practices to implement preventive interventions and accurate Braden risk assessments on admission.

Job description

Objective

A registered licensed nurse designated to act as the coordinator of the center's wound team to oversee the general application of the skin management program. The following activities are performed as directed or clinically indicated.

Principal Responsibilities

• Identify and maintain effective methods to gather pressure ulcer and skin alteration related information
• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation
• Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare practitioners
• Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions

Qualifications

• Currently licensed as RN/LVN/LPN in state

One-year prior nursing management experience preferred
