Description

Treatment Foster Care Coordinator

Pay: $45,000

This is a community-based position. Travel and home visits are required.

Treatment Coordinators with NYAP work in the community as part of an integral service team. Under the supervision of the Clinical Supervisor or designee, this role works closely with youth, family, foster parents, and community partners, providing advocacy and support to help youth live safely and successfully in the community.

Responsibilities:

  • Provide individual, group, and family mental health services, along with other evidence-based clinical services.
  • Travel daily to deliver community-based services to youth and families, complying with organizational, contract, and regulatory standards.
  • Coordinate and monitor clinical services, including mental health, educational support, medication management, and psychological services.
  • Review and adapt youth-based plans regularly.
  • Build and maintain strong relationships with foster parents and biological families.
  • Assist in family reunification and stabilization efforts in line with the youth’s case plan.
  • Provide safety assessments, crisis response, and behavioral stabilization services as needed.

Minimum Qualifications:

  • Bachelor’s Degree in Social Work from an accredited institution.
  • LSW license-eligible.
  • Preferred: 2 years of direct service experience with youth and families.
  • Willingness to work flexible and non-traditional hours.
  • Proficiency with computers, smartphones, tablets, and relevant software.

Driving and Vehicle Requirements:

  • Valid driver’s license
  • Reliable transportation
  • Good driving record
  • Minimum auto insurance coverage of $100,000/$300,000 bodily injury liability
