Job summary

Intercept Health in Virginia seeks a full-time Foster Care Family Consultant to provide casework services for children and families. Reports to the TFC Program Supervisor and requires a degree with relevant experience in social work or related fields.

The role includes home studies, service planning, and coordination across medical, social, and educational services to support placement or discharge. Competitive benefits are offered with full-time employment.

Qualifications

• Bachelor’s degree plus two years’ casework with children/families or
• Bachelor’s in social work (or related field) with one year of casework
• Master’s/Doctorate in social work with placement or related experience

Responsibilities

• Interview children and families.
• Conduct and write home studies.
• Develop individualized treatment and service plans.
• Counsel children and families in placement/pre-discharge.
• Supervise children in various foster/adoptive settings.
• Prepare and maintain case records.
• Coordinate services to ensure continuous access to care.
• Assess ongoing needs for psychosocial, medical, educational services.
• Coordinate referrals and link child to needed services.
• Monitor progress and review plans with involved parties.
