Overview

Location: West Valley Behavioral Health Center

Position: Treatment Coordinator

The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing service needs, including both primary care and behavioral health. This position helps clients and families obtain community support, identify resources, and resolve barriers.

Responsibilities

1. Coordinate services for children and adults in accordance with treatment plans.
2. Respond to initial crisis calls and identify needs and resources accordingly.
3. Engage formal and informal resources for the client, including transition, discharge, and aftercare plans of behavioral health services, demonstrating teamwork and shared leadership.
4. Coordinate behavioral health services with other agencies such as DCS, Probation/Parole, DDD, AZ Long Term Care, schools, and providers.

Qualifications

1. Bachelor's Degree in a behavioral health field preferred; experience and education may be considered.
2. Relevant experience in a behavioral health setting.
3. Level 1 Fingerprint Clearance Card.
4. Verification of education, employment, and references.
5. Valid AZ Driver's License, auto insurance, and MVR matching JFCS guidelines.
