Role Overview

The hospice aide is responsible for providing personal care under the direction of a registered nurse.

• $18.00-$22.00

Responsibilities

• Document care provided according to organization policy.
• Report any deviations from the Plan of Care to the Case Manager.
• Use Durable Medical Equipment safely.
• Take vital signs as outlined in the Hospice Aide Plan of Care.
• Foster cooperative effort among personnel by understanding the functions of others involved in patient care and participating actively in team and staff conferences.
• Provide proper care and observation of patient's skin to prevent tissue breakdown over bony prominences. Report any changes or areas of concern to the Case Manager.
• Assist with ambulation, transfer, and range of motion as documented on the Hospice Aide Plan of Care.
• Provide respite for patients' family/caregivers when on-site, as appropriate.
• Participate in QAPI activities.
• Participate in IDG meetings, as appropriate.

Qualifications

• Meet training requirements by the state of Arizona.
• One year of experience as a CNA, hospice aide, or home health aide.
• At least 18 years old.
• Must be a licensed driver with an insured automobile in good working order.
• Effective verbal and written communication skills.
• Ability to work as part of a team and independently.
