Job summary

Jewish Family and Children's Service of Arizona is seeking a Treatment Coordinator to provide ongoing care-coordination for clients with behavioral-health needs. You will address barriers, help secure community resources, and support families according to treatment plans.

Responsibilities include crisis response, engaging formal and informal resources, and coordinating with agencies like DCS, probation, and schools. A human services background and AZ driving credentials are required.

Qualifications

  • Bachelor's Degree in a human services field preferred; a combination of experience and education may be considered.
  • Relevant experience working in a behavioral-health setting.
  • Verification of education, employment, references.
  • MVR matching JFCS driving guidelines.
  • AZ Driver's License and Valid Auto Insurance.

  • Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.

Treatment Coordinator:The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family's ongoing behavioral health needs. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.
