The role requires an approved nursing degree, state licensure, and the ability to lead care coordination efforts with an interdisciplinary team in a VA setting.

Qualifications

• English proficiency required; basic written and spoken English standards apply.
• Graduate of nursing program approved by ACEN or CCNE; state licensure required.
• Bridge program path may allow licensure prior to completion; temporary appointment possible.
• Foreign nursing graduates must have current, full, active, unrestricted registration in a US state or territory.
• License verification and degree equivalency may be required for foreign education.

Responsibilities

• Coordinate care for veterans requiring complex care coordination or case management.
• Serve as primary point of contact for veterans and families; communicate with health care teams.
• Lead complex care coordination and ensure timely access to care across settings.
• Collaborate with interdisciplinary team to align care with professional nursing standards.
• Promote a safe, supportive environment and mentor peers.
