Job summary

The U.S. Department of Veterans Affairs is seeking a Traveling Veteran Coordinator (TVC) Registered Nurse (RN) to streamline care delivery for Veterans requiring complex coordination and specialized case management.

The TVC travels to patient sites as needed and serves as the primary contact for Veterans and families, coordinating with the health care team. As a licensed RN, you will lead complex care coordination efforts, ensure timely triage, and direct services in alignment with VA standards

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a school of professional nursing approved by accrediting bodies (ACEN/CCNE) or eligible for licensure via bridge program.
• Current, full, active, and unrestricted nursing licensure in a US state/DC.
• GNT exception: temporary appointment while pursuing licensure may be offered.

Responsibilities

• Receive, triage, and direct timely, coordinated care for Veterans.
• Serve as the primary contact for Veterans and their families or caregivers.
• Communicate with the health care team while leading complex care coordination and case management.
