Job summary

Jefferson Health is seeking a RN for Home Infusion to provide individualized nursing care in patients' homes and coordinate treatment plans. The role requires travel to patient locations and collaboration with the care team to ensure quality outcomes.

Responsibilities include patient assessments, initiating IV therapy, and timely documentation. A valid RN license, BLS, and a driver's license are required; hospital experience preferred.

Qualifications

• Graduate of an accredited nursing program and licensed RN.
• Current BLS certification and valid driver's license.
• Experience in acute care or home/hospice infusion preferred.

Responsibilities

• Plans for, coordinates, and provides individualized nursing care in patients' homes.
• Performs patient assessments and implements treatment plans.
• Initiates, maintains, and discontinues IV therapy with clients.
• Submits documentation within 48 hours and communicates with the health care team.
