Jefferson Health Plans, a not-for-profit health plan, seeks a licensed RN for Home Infusion to plan, coordinate, and provide individualized nursing care in patients' homes. This role reports to Care Team Leader/Nurse Manager and involves travel to patient locations on a regular schedule.

Requirements include graduating from an accredited nursing school, RN licensure in the state, and 2+ years in acute care or 1 year in home/hospice/home infusion settings.

Competenze

• Graduate of accredited nursing school.
• At least 2 years in the acute care setting or 1 year in home care, hospice or home infusion setting.
• Current RN licensure in state of employment.

Mansioni

• Performs patient care in a timely and efficient manner.
• Maintains a positive impact on customers (internal and external).
• Performs patient assessments as required by patient condition and/or agency policy.
• Provides client education for clients/caregivers receiving IV therapy at home.
• Communicates with other members of the health care team on issues pertaining to client care needs, status updates, and coordination of care issues.
• One weekend shift every other weekend; travels to patient homes/visit locations.
