Job summary

Jefferson Health Plans seeks an RN for Home Infusion to plan, coordinate, and provide individualized nursing care at patients' homes. You will collaborate with the Care Team Leader or Nurse Manager to ensure high-quality care across visits and assessments.

The role requires an accredited nursing degree and current RN licensure in the state of employment, with a typical day shift and travel between patient homes. Weekend coverage occurs every other week within a full-time schedule.

Qualifications

• Graduate of accredited nursing school.
• At least 2 years in acute care or 1 year in home care, hospice or home infusion setting.

Responsibilities

• Performs patient care in a timely and efficient manner.
• Maintains a positive impact on customers (internal and external).
• Performs patient assessments as required by patient condition or agency policy.
• Provides client education for clients/caregivers receiving IV therapy at home.
• Communicates with other health care team members on care needs, status updates and coordination.
• Primary shift 10a-6:30p with options for 10h or 8h slots.
• One weekend shift every other weekend.
• Employee travels to patient homes/visit locations.
