Guardian Pharmacy, LLC in Eugene, OR seeks a licensed Pharmacist to review nursing-home drug regimens, ensure compliant, optimal therapy, and serve as a liaison with facilities. You will collaborate with physicians and the Guardian team to deliver high-quality medication management.

This role requires a BS/PharmD, an active Pharmacist License, and up to 90% travel. You will train staff, review orders, and provide monthly reports in a fast-paced, team-driven environment with strong attention to

Qualifications

• BS/PharmD degree required and active Pharmacist License.
• Valid driver's license and clean driving record required.
• 2+ years of related LTC or high-volume pharmacy experience preferred.
• Advanced computer skills; pharmacy operations system experience required.
• Ability to work independently and meet deadlines; detail-oriented.

Responsabilités

• Evaluates drug regimens in nursing facilities to assure appropriate therapy.
• monitors facility compliance with Federal and State guidelines and notes irregularities.
• Serves as an information source regarding medication and regulatory guidelines.
• Liaises between the pharmacy and facility to resolve problems.
• Educates nurses on medication areas and staff interests.
• Attends facility, infection control, QA, and inter-disciplinary meetings.
• Reviews physician orders for action, dosage, interactions, and stability.
• Trains/supervises pharmacy technicians, interns, and externs.
• Produces monthly reports for facilities and provides to Administrator/Medical Director.
• Reconciles controlled drugs and audits patient charts.
