Required BS/PharmD, active pharmacist license, and a valid driver's license. Expect independent work, strong attention to detail, and the ability to handle multiple tasks in a fast-paced, team-driven environment.

Qualifications

• BS/PharmD degree in pharmacy is required.
• Pharmacist license is required.
• Valid driver's license and clean driving record.
• 2+ years of LTC or high-volume pharmacy experience.
• Advanced computer skills and pharmacy operations system experience.
• Ability to work independently and meet deadlines.
• Detail-oriented with strong accuracy.
• Able to work in a fast-paced, team environment handling multiple tasks.

Responsabilités

• Evaluates the drug regimens of patients in nursing homes to assure appropriate therapy.
• Monitors facility compliance with Federal and State guidelines and notes irregularities.
• Serves as information source regarding medication and regulations.
• Acts as liaison between the pharmacy and facility to resolve problems.
• Educates nurses about problem areas and staff education.
• Attends meetings of facility, pharmacy, infection control, QA, and interdisciplinary teams.
• Reviews physician orders for action, dosing, interactions, administration, and stability.
• Assists in training and supervising pharmacy technicians, interns, and externs.
• Produces written reports of monthly visits and shares with administrators and medical directors.
• Maintains seizure and drug problem notice files.
• Reconciles controlled drugs and audits patient charts.
• Other essential duties as needed.
