Job summary

Consonus Pharmacy is seeking a Consultant Pharmacist to support long-term care facilities in the Eugene area. You will review resident regimens, provide education to staff, and ensure compliance with policies, CMS guidelines, and QA processes.

Travel within the territory, including overnight stays, is required. A PharmD or BS Pharm and active license are essential to join a dedicated team focused on patient-centered care and collaboration with facilities and providers.

Qualifications

• B.S. Pharm. or Pharm D. from an accredited School of Pharmacy.
• Current Pharmacy license in the state(s) of practice
• Ability to travel throughout a territory, including overnight stays as needed

Responsibilities

• Review resident medication regimens monthly per patient acuity level and report findings in writing to facility administrator, director of nursing and physician
• Plan and present in-service education to other pharmacy staff and nursing facility employees as requested
• Understand and implement all appropriate pharmacy policies & procedures
• Participate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administration
• Liaison between facility and pharmacy staff
• Provide ongoing education directly or indirectly to residents preparing for discharge from facilities serviced
• Knowledge of CMS regulations and interpretive guidelines
• Attend quality assurance and other appropriate committee meetings as requested by facilities
• Prepare quarterly quality assurance reports
