Zusammenfassung

Compassus is seeking a Traveling Registered Nurse to provide homecare–based nursing services across various programs. The role emphasizes compassion, integrity, excellence, teamwork, and innovation, with assignments typically lasting 8 to 13 weeks.

You will perform routine and urgent assessments, educate patients and caregivers, and coordinate care to meet patient needs and ensure comfort. As a team member, you will collaborate with the Hospice IDT, maintain documentation, and travel extensively

Qualifikationen

• Associate or Bachelor's degree in Nursing required.
• Minimum of one year of nursing practice or equivalent experience preferred.
• Home Health, hospice, oncology, and/or long-term care experience preferred.

Aufgaben

• Travel to assigned program based on need; assignments 8-13 weeks (about 3 months).
• Admits patients and performs routine and emergency assessments.
• May act as preceptor to onboard and train new team members.
• Meets productivity standards and is available for visits during the regular work day.
• Coordinates clinical disciplines per Plan of Care through case management.
• Ensures bedside documentation of observations and treatments.
• Communicates with IDT, Medical Director, and Attending Physician as directed.
• Prevents unwarranted hospitalizations by meeting patient and family needs at home.
• Coordinates hospice/home health care to ensure continuity and quality of care.
• Develops and updates care plans after each POC change.
• Educates patients and caregivers regarding care, disease, and IV therapy.
• Completes referral and admission assessments.
• Supervises care provided by LPNs and HHAs.
