Job summary

Elara Caring is seeking a Home Health Physical Therapist to deliver patient-centered care in Niles, Michigan. This staff position offers hourly pay of $41-$58 and a pathway to growth within a nationwide home health network.

You’ll travel within the branch area up to 50% and provide high-quality therapy in patients’ homes, coordinating with care teams for optimal outcomes. The role requires an APTA-approved PT program graduate, an active PT license in Michigan, and at least one year of healthcare

Qualifications

• Graduate of an APTA-approved PT program.
• Current, unrestricted PT license for Michigan.
• Minimum one year of healthcare facility experience.
• Willing to travel up to 50% of the time.
• Able to lift 50–100 lbs and perform transfers.

Responsibilities

• Provides direct patient care and evaluates functional needs and outcomes.
• Designs and implements a plan of care based on assessments.
• Collaborates with other specialists to optimize patient outcomes.
• Directs support staff to achieve patient goals.
• Maintains documentation of evaluations, goals and plans.
• Contributes to community physical therapy leadership and education.
