Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home-based care team in Michigan. You will evaluate patients, design individualized treatment plans, and deliver therapy in patients' homes, coordinating with physicians and caregivers.

Must hold an APTA-approved PT program degree, an unrestricted state license, and a valid driver’s license. At least one year of healthcare facility experience is required, with the ability to lift up to 50 pounds and travel between homes.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved PT program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license, or reliable transportation to travel between patient homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
