Your Health Organization is seeking a Registered Nurse to service patients throughout the Spartanburg area. The Visiting Nurse (Nurse Case Manager) will travel to patients’ homes and facilities to evaluate care and initiate telemedicine visits as part of the care team.

Responsibilities include coordinating with providers, enrolling patients in Care Management, and ensuring accurate documentation and safe home environments. On-call duties may be required.

Qualifikationen

• Licensed registered nurse in good standing
• Minimum 3 years clinical experience preferred
• Experience in community settings preferred
• Strong written and verbal communication
• Ability to work with interdisciplinary teams
• Flexible hours and travel required

• Initiate and manage Care Management Plan of Care
• Coordinate and document Care Management activities
• Reconcile medications and ensure safety in home environments
• On-call may be required
