Job summary

Sanford Health is seeking a Nursing Assistant (NA) to provide resident-centered care in long-term care settings. The NA assists with daily living activities, vital signs, and basic health care needs under RN supervision, while following care plans and infection control policies.

The role requires CNA certification, high school diploma or GED, and at least 18 years old. Travel may be required within a 75-mile radius of the permanent address. Competitive premium pay and shift differentials apply.

Qualifications

• High school graduate or GED preferred but not required.
• Minimum of 18 years of age.
• Prior clinical or nursing assistant experience in long-term care preferred.
• CNA certification with the State Board of Nursing, or state certified CNA within four months.
• CO: QMAP certification may be required.
• Annual continuing education requirements (12 hours) and annual competency testing.

Responsibilities

• Assist residents with daily living activities and vital signs.
• Provide resident-centered nursing care and psychosocial support.
• Assist with transfers, repositioning, and ROM exercises.
• Support meals, dining, hydration, and medication assistance as appropriate.
• Document resident interactions and communicate changes to nursing staff.
