Job summary

Easterseals PORT Health seeks a compassionate Tailored Care Manager to assess client needs, build personalized care plans, and coordinate health, social, and practical supports across North Carolina communities.

This full-time role involves travel to Greenville, NC and Winterville, NC, a Monday–Friday schedule from 9 AM to 5 PM, with rotating on-call duties, and a competitive compensation package.

Qualifications

• Bachelor’s degree in health, psychology, sociology, social work, nursing or related field.
• QMHP credentialing eligible; Master’s degree preferred.
• Minimum 2 years of experience with mental health/substance use populations.
• Proficiency with MS Office and EHR systems.
• Valid driver’s license and clean driving record.
• Strong communication and ability to work independently.

Responsibilities

• Evaluate client needs and develop personalized care plans.
• Coordinate services addressing health, social determinants, and life challenges.
• Travel throughout Greenville, NC and Winterville, NC to close care gaps.
• Participate in rotating after-hours support as needed.
