Qualifications

• Master’s degree in a human services field.
• RN license or LCAS/LMFT/LCMHC required.
• Minimum 3 years of experience in care management or related field.
• 1 year of supervisory or team lead experience preferred.
• Valid driver’s license and insurance for work travel.

• Mentor and support Care Managers to deliver high-quality, person-centered services.
