Job summary

Easterseals PORT Health in Pitt County, NC seeks a Tailored Care Management Supervisor to lead a dedicated care team delivering whole-person, person-centered services. You’ll provide strategic oversight, mentor care managers, and drive quality improvement while fostering collaboration across programs.

Qualifications

• Master’s degree in a human services field.
• Full or provisional license: LCAS, LMFT, LCSW, LCMHC, or RN (REQUIRED)
• At least 3 years of experience in care management, coordination, or case management
• Minimum of one year supervisory or team lead experience preferred
• Strong communication, crisis de-escalation, and strategic problem-solving skills
• A valid driver’s license and insurance for work travel

• Mentor and support Care Managers to deliver high-quality, person-centered services.
• Drive program excellence through strategic planning, quality improvement, compliance, and fiscal oversight.

• Your leadership will ensure that those we serve feel seen, heard, and empowered to take control of their journey forward.
