Responsibilities

• Assess patient needs and signs/symptoms; use nursing process to plan, implement and evaluate care.
• Collect, analyze, and document data and nursing diagnoses.
• Document plan of care with expected outcomes and interventions.
• Initiate and evaluate patient and/or family education.
• Revise and document the care plan based on evaluation and changes in medical plan.
