Job summary

HealthForce Healthcare seeks a Speech Language Pathologist for an Acute Care travel assignment in Canaan, CT. The role runs for 13 weeks starting 11/30/2026 with guaranteed 32 hours per week on a 5x8 day schedule (08:00–16:30) in a long-term care/nursing home setting.

Responsibilities include comprehensive speech-language assessments, treatment planning, direct therapy, documentation, and collaboration with an interdisciplinary team.

Qualifications

• Ability to work in a long-term care/nursing home.
• Day shift availability 08:00–16:30.
• Strong documentation and communication skills.
• Willingness to participate in a 13-week travel assignment.

Responsibilities

• Conduct comprehensive speech-language assessments and screen for swallowing disorders in residents.
• Develop and implement individualized treatment plans addressing communication, cognitive-communication, and swallowing goals.
• Provide direct speech-language therapy and monitor progress; adjust plans as needed.
• Document assessments and treatment notes in the medical record; communicate findings to the care team.
• Collaborate with an interdisciplinary team to optimize patient outcomes and discharge planning.
• Educate residents and families on therapy goals, home practice programs, and safety strategies; participate in care conferences.
• Maintain equipment, follow infection control protocols, and ensure safety during therapy sessions.
