Job summary

Personal Touch Home Health Care Services Inc. seeks a Travel Skilled Nursing Facility (SNF) Physical Therapy Assistant (PTA) to support patient mobility and recovery. You will work under a licensed Physical Therapist and follow treatment plans to help patients regain movement.

Responsibilities include documenting progress, assisting with transfers, gait training, ADL-focused interventions, and providing therapy in multiple SNFs across New York.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active NY State PTA license.
• BLS certification.
• Preferred experience in skilled nursing or long-term care settings.
