• Collect and analyze patient assessment data to determine diagnoses and care plans.
• Implement care plans, coordinate care delivery, and promote a safe healing environment.
• Collaborate with patients, families, and the healthcare team to provide safe and compassionate care.
• Participate in discharge planning to ensure continuity of care.
• Provide learning opportunities and health information to patients, families, and team members.
