American Traveler, LLC. is seeking a Hospice RN to lead an interdisciplinary care team across home, nursing homes, and assisted living settings in Colorado.

A CO or compact RN license and 2+ years of nursing experience are required. The role involves supervising care in patients\' homes and facilities, coordinating goals of care, educating caregivers, and delivering compassionate hospice and palliative services within a mission-driven team.

Qualifications

• Active CO or compact RN license required.
• Minimum of 2 years of nursing experience required.
• Strong clinical assessment and critical thinking skills required.
• Generalist knowledge across disease processes with specialization in symptom management.

Responsibilities

• Weekday schedule Monday through Friday, 8am to 5pm
• Occasional back-up coverage required for weekends and on-call
• Care provided in patients' homes, nursing homes, group homes, or assisted living facilities
• Primary RN carries an assigned caseload and leads the interdisciplinary care team
• Serve as both clinical caregiver and teacher, helping patients set goals of care and educating caregivers
• Provide compassionate, personalized hospice and palliative care
• Work within a supportive, mission-driven hospice environment
