American Traveler, LLC. is hiring an RN Case Manager for a Hospice position in Loudoun County, VA. The role requires an active VA or Compact RN license and at least 1 year of hospice/case management experience.

You will provide home-based hospice case management with an average caseload of 5-7 patients per day, day shifts, and travel within a 60-mile radius.

A Netsmart EMR is used; scrubs are required; iPad provided; mileage reimbursement and housing support offered.

Qualificações

Responsibilities

• Hospice setting providing case management services in patients' homes within Loudoun County, VA
• Geriatric end-of-life care patient population
• Average caseload of 5-7 patients per day
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
• No on-call responsibilities
• Travel within up to a 60-mile radius from the hub office required
• Uses Netsmart EMR
• Responsible for timely documentation and communication with the care team
• Must be flexible with assigned territory
• Scrubs required in any color, no jeans permitted
• Cell phone use not permitted; iPad provided for work purposes
• Mileage reimbursement provided
• No RTO permitted during the first two weeks or during orientation
• Local candidates accepted at the travel rate
