Job summary

American Traveler is seeking an experienced RN for home hospice care in New Jersey. The role requires an active NJ RN license or compact license, BLS, and experience in a home hospice setting; Epic EMR is used for charting.

The position features a 48-hour weekly schedule with PM shifts, on-call coverage for Morris & Sussex Counties, weekend rotation, and potential travel to sister locations. Candidates should have strong critical thinking, communication skills, and knowledge of hospice

Qualifications

• Active NJ RN license or compact license required.
• Valid NJ driver's license required.
• BLS certification required.
• Minimum 2 years of RN experience in a home hospice environment required.
• ASN or diploma with NJ RN license required.
• 3+ years of RN experience preferred.
• Experience in hospice agency or organization preferred.
• Experience using an EMR required.
• Strong critical thinking and communication skills required.
• Knowledge of hospice regulations, pharmacology, and safe medication management.

Responsibilities

• Home care setting within a Hospice and Palliative Care program
• PM shift, 4pm to 8am, 3 shifts per week (16 hours per shift, 48 hours weekly)
• On-call coverage required for Morris & Sussex Counties
• Weekend rotation required every 3 weeks with comp days provided
• One summer and one winter holiday coverage required
• May float to sister hospital location as needed
• Uses Epic EMR for charting
• Performs patient assessments to determine hospice appropriateness and develops individualized plans of care
• Completes admission, discharge, and re-visit documentation with timely data transfer
• Provides age-appropriate patient and family education and evaluates teaching effectiveness
• Manages caseload including scheduling, care plan revisions, and interdisciplinary communication
• Attends IDT meetings every 2 weeks and monthly staff meetings
• Supervises HHAs according to hospice standards
• Maintains a safe environment through infection control and medication reconciliation practices
• Participates in performance improvement activities and may mentor new employees
• Local candidates accepted at a lower rate
• Travelers may stay on assignment up to one calendar year from initial start date before requiring a break
