Caris HealthCare in Murfreesboro, TN is seeking a Registered Nurse Travel RN PRN to provide nursing care per physician orders for patients in Hospice/Home Health settings. Pay ranges from $30.00 to $45.00 per hour, depending on experience and responsibilities.

The role encompasses assessment, planning, implementation, and evaluation of total patient care, with a focus on patient advocacy, teaching, and interdisciplinary collaboration.

Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience required, Hospice or Home Health preferred.
• Sound knowledge of nursing principles and patient teaching.

Responsibilities

• Obtains a medical history and assesses the patient’s physical and emotional status, home environment, safety factors, and teaching needs.
• Advocates for patients and involves family in developing Plan of Care.
• Updates interdisciplinary care plan every two weeks and implements interventions to reach goals.
• Supervises care provided by Hospice Aide/CNA and evaluates it per guidelines.
• Observes signs and informs physician/PCC of any changes in condition.
• Participates in Performance Improvement activities.
• Participates in on-call rotating schedule and provides back-up support.
• Uses electronic medical records documentation.
