Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience required, with experience in Hospice or Home Health preferred.
• Must possess a sound knowledge of nursing principles and patient family teaching.

Responsibilities

• Assess and monitor patient’s symptoms using required software.
• Collaborate with interdisciplinary team to develop individual care plans.
• Serve as patient advocate providing information on patient rights.
• Evaluate patient’s status and determine education needs for caregivers.
• Supervise and direct aides and licensed practical nurses.
• Participate in on-call rotating schedule.
