Silverado Hospice is hiring a Registered Nurse (RN) to travel around Los Angeles County for admissions evaluations, initiating care plans, and ensuring smooth transitions for patients entering hospice. The role requires an active RN license and at least 1 year of nursing experience, with willingness to drive and work outside regular hours.

You will coordinate services across settings, guide families, and document within 24 hours of visits, while upholding compassionate, patient-centered care.

Qualifications

• Must be a Registered Nurse with a valid state license.
• Minimum of 1 year nursing experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Must clear background checks and drug screen.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive nursing evaluations to admit patients into hospice.
• Assess signs of discomfort and take prompt, appropriate action.
• Initiate care plans and provide nursing follow-up after hours.
• Instruct caregivers on managing patient needs and submit documentation within 24 hours of visit.
• Coordinate all services across settings to ensure quality and appropriateness of care.
• Follow approved plans of care, modify as needed, and communicate with physicians when appropriate.
