Job summary

You lead with clinical expertise and empathy, delivering dignified care in a mission-driven environment and coordinating with physicians, caregivers, and the care team to maintain high-quality outcomes.

Qualifications

• Valid state RN license
• Minimum 1 year nursing experience; hospice experience preferred
• Ability to lift up to 50 pounds
• Must clear criminal background check, physical, and drug screening
• Valid driver’s license with reliable transportation
• Willingness to drive to patient locations and work outside regular hours

Responsibilities

• Perform comprehensive nursing evaluations to admit patients into hospice
• Assess for signs and symptoms of discomfort and take prompt action
• Initiate care plans and provide nursing follow-up after hours
• Instruct caregivers on patient needs and submit documentation within 24 hours of visit
• Coordinate services across settings to ensure quality and appropriateness of care
• Follow plans of care, modify as needed, and communicate with physicians
