Silverado Hospice is hiring a Registered Nurse to travel around Los Angeles County for admissions. You will perform comprehensive nursing evaluations, initiate care plans, and ensure a smooth transition for patients entering hospice, supporting families in critical moments.

The role requires an active RN license, at least 1 year of nursing experience (hospice preferred), and the ability to lift 50 pounds. A valid driver’s license and reliable transportation are musts for this travel-heavy

Qualifications

• RN with a valid state license.
• Minimum of 1 year nursing experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Must pass background check, physical, and drug screen.
• Valid driver’s license and reliable transportation; willing to drive to patient locations.

Responsibilities

• Perform comprehensive nursing evaluations to admit patients into hospice.
• Assess for signs of discomfort and take prompt action.
• Initiate care plans and provide nursing follow-up after hours.
• Instruct caregivers on patient needs and submit documentation within 24 hours of visit.
• Coordinate services across settings to ensure quality and appropriateness of care.
• Follow plans of care and communicate with physicians as needed.
