American Traveler, LLC. is hiring a Home Health RN with OASIS and HCHB experience to support a geriatric patient caseload in the Walnut Creek area. The role involves travel within the service area, 30 patient points per week, and a day shift schedule.

Active CA RN license and BLS are required, with 1 year of Home Health RN experience and OASIS/HCHB experience preferred. Mileage reimbursements and scrubs provided. First-time travelers welcome and local candidates accepted at travel rate.

Formación

• Active CA RN license required at time of consideration.
• Basic Life Support certification required.

Responsabilidades

• Serve Walnut Creek, Lafayette, Orinda, Moraga and San Ramon area within a 60-mile radius.
• Provide care for a geriatric patient population.
• Manage caseload of ~30 patient points per week (5–6 daily).
• Work day shift 8-hour, 8:00 AM–5:00 PM; two weekend days per month.

American Traveler is hiring a Home Health RN with OASIS and HCHB experience for a geriatric patient caseload in the Walnut Creek area.

Requirements

• Active CA RN license required at time of consideration
• BLS certification required
• Minimum 1 year of recent Home Health RN experience required
• OASIS experience required
• HCHB (Homecare Homebase) experience required
• Experience with admission assessments, recertifications, resumption of care, and routine visits
• Wound care and integumentary skills required
• Medication administration and management experience, including IM, IV, and subcutaneous routes
• IV starting, maintenance, and phlebotomy skills required
• Infection control precautions knowledge required

Responsibilities

• Home Health setting serving Walnut Creek, Lafayette, Orinda, Moraga, and San Ramon (up to a 60-mile radius)
• Primarily geriatric patient population
• Caseload of approximately 30 patient points per week (5-6 points per day)
• Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
• Two weekend days required per month
• No on-call requirements
• 40 guaranteed hours contingent on meeting productivity requirements
• Responsible for timely clinical documentation and communication
• Scrubs required (any color), jeans not permitted
• iPad provided for documentation; cell phone not provided
• Mileage reimbursed at the current IRS standard mileage rate
• Travelers should be flexible with assigned territory within the service area
• First-time travelers are welcome to apply
• Local candidates accepted at the travel rate
