Job summary

Tempus Unlimited, Inc. is seeking a Registered Nurse to support a Community-based Personal Care Management Evaluation program.

The role requires travel across the MA coverage area and the Weymouth vicinity, with a 35-hour work week and a 2-day orientation in Stoughton. Ideal candidates hold a MA RN license, have at least 2 years of direct care experience with disabled or elderly individuals, and can commute reliably.

Qualifications

• Current Massachusetts RN license in good standing.
• Minimum 2 years of direct care experience with elderly or disabled individuals.
• Must have a valid driver’s license and reliable transportation.
• Flexible schedule with community-based travel within MA.

Responsibilities

• Perform initial evaluations and annual reevaluations per MassHealth 130 CMR 422.422(C).
• Submit completed evaluations within required timeframes.
• Schedule reevaluations and update calendars.
• Respond to hours adjustment requests and MassHealth inquiries.
• Maintain progress notes and coordinate with PCA Eval Dept Assistants.
• Work with PCA Program team and participate in monthly meetings.
