Job summary

Longevity Health is seeking an RN Field Coordinator based in Glasgow, KY, to support Longevity Health Plan members and nursing facilities within the assigned territory. The role involves on-site visits, data gathering, and facilitating onboarding for new members and facilities.

The RN will educate clinical staff on the Longevity Health Plan model of care, coordinate member transitions, and help initiate telehealth services across onboarded facilities.

Qualifications

• Licensed registered nurse in good standing.
• Minimum 2 years of RN experience preferred.
• Ability to travel and work across multiple nursing home facilities.
• Proficient in EMR/clinical platforms and standard MS Office tools.

Responsibilities

• Conduct in-person visits at nursing homes to support clinicians with clinical and administrative tasks.
• Use EMRs to prescreen enrollees and identify continuity of care needs.
• Coordinate with clinical and operations teams to manage member appointments and communications with facilities.
• Support transitions of care and assist with prior authorizations and access to services outside facilities.
• Lead orientation for facilities to educate staff on Longevity Health Plan model of care.
