Job summary

Longevity Health is seeking an experienced RN Field Coordinator to join our team. You will travel to nursing homes, support Advanced Practice Providers, and coordinate transitions of care for plan members within your market.

Responsibilities include coordinating with clinical staff, using EMR data to guide onboarding, and educating facilities on the Longevity Health Plan model of care. Travel within the territory is required, with flexible scheduling.

Qualifications

• 2+ years of RN experience required.
• Active RN license in good standing.
• Valid state driver’s license with insured vehicle.
• EMR/clinical platform and MS Office proficiency.

Responsibilities

• Conduct in-person visits at nursing homes to assist APPs with clinical tasks.
• Prescreen enrollees using facility EMRs to identify care needs.
• Collaborate with clinical and operations teams to coordinate member care.
• Support transitions of care and prior authorization requests.
• Lead orientation for staff on Longevity Health Plan model of care.
• Help initiate telehealth services in onboarded facilities.
• Maintain proficiency in Longevity EMR and assist with data entry.
