Job summary

Longevity Health Plan is seeking an RN Field Coordinator to support institutional special needs members across nursing homes in the Glasgow, KY area. This licensed RN will educate facility staff on the Longevity Health Plan model, gather data for onboarding, and ensure smooth member transitions with EMR access and care coordination.

The role requires travel, strong communication skills, and experience in care coordination within long-term care settings.

Qualifications

• Active registered nurse license in good standing.
• Associate degree in nursing required; bachelor’s degree preferred in nursing or related field.

Responsibilities

• Conduct in-person nursing home visits to support clinicians and facilities.
• Prescreen enrollees using EMR to identify continuity of care needs.
• Oversee member appointments and communicate with providers and facility staff.
• Assist with transitions of care and prior authorization processes.
• Lead orientation for new and existing nursing homes on Longevity Health Plan model.
