Job summary

Longevity Health is seeking an RN Field Coordinator to blend patient care with administrative coordination in long-term care settings. You will travel to nursing homes upon assignment by the Market Director of Clinical Services, supporting transitions of care and staff education on the Longevity Health Plan model of care.

Responsibilities include prescreening enrollees in EMRs, coordinating member appointments, and assisting with telehealth initiation and data entry.

Qualifications

• Excellent verbal and written communication skills.
• Proficient with EMR/clinical platforms and MS Office.
• Willingness and ability to travel to multiple nursing facilities.

Responsibilities

• Conduct in-person visits at nursing homes to support APNs with clinical and administrative tasks.
• Prescreen new enrollees in EMRs and identify continuity of care needs.
• Coordinate appointments and ensure timely communication with providers and facility staff.
• Assist with transitions of care and prior authorization requests.
