Job summary

American Traveler is seeking an RN Care Coordinator for a Case Management/Utilization Review role requiring a California RN license and prior travel nursing experience. The position involves night shifts (3x12h) and coordination of care for medically vulnerable patients within a hospital setting.

Candidates must have EPIC or EPIC/Cerner experience, BLS certification, and at least 2 years of clinical experience. Travel nursing background and supervisory references are required.

Qualifications

• Active California RN license required.
• Current AHA BLS certification required.
• 2+ years of clinical experience required.
• 1 year total travel nursing experience required.
• 1 supervisory-level professional reference required.
• EPIC or combined EPIC/Cerner experience within the last 6 months required.

Responsibilities

• Care Coordinator unit within a hospital setting.
• Night shift schedule, 3x12-hour shifts from 19:00 to 07:30.
• Screens assigned caseload to identify medically vulnerable patients and coordinate care.
• Coordinates with healthcare team and patient/family to develop a transition plan of care.
• Uses EPIC EMR system.
• Discharge/transition planning for medically vulnerable patients.
